How to Choose The Right Breast Cancer Surgery

Once you have been diagnosed with breast cancer, usually treatment starts first with surgery. There are generally two components to the operation: 1) the breast, and 2) the axilla (armpit).

In the breast, there are two choices: 1) lumpectomy (removing the cancer lump and some surrounding tissue), or 2) mastectomy (removing the entire breast). Just a quarter of a century ago, almost all patients had mastectomy, even if their breast cancer is small. But over the past few decades, it has been demonstrated that for small cancers, lumpectomy followed by radiation treatment is just as good as mastectomy. Now, “small” is a relative term. The goal of a lumpectomy is that the breast would still look like a breast after all the treatment is done. In someone with small breasts, a “small” cancer may occupy a significant portion of her breast. An adequate lumpectomy in this case may result in an unacceptable cosmetic outcome, and the patient may actually prefer having mastectomy with plastic surgery reconstruction. Another factor is the number of cancers in the breast. Usually, mastectomy is required for multifocal cancer (cancer discovered in two or more locations in the same breast)

At the same time as the breast operation, surgery in the axilla also is carried out for invasive (or infiltrating) breast cancer, as well as in selected cases of DCIS (ductal carcinoma in situ). This is done to determine whether the breast cancer has spread to the lymph nodes in the axilla. In general, there are two choices: 1) sentinel node resection, or 2) axillary node dissection. In sentinel node resection, the surgeon maps the lymphatic drainage of the breast cancer to the first lymph node that receives this lymph, i.e. the sentinel node(s). Only a few (1-3) nodes are then usually removed. In axillary node dissection, more lymph nodes are resected, usually 8-25. Sentinel node resection is a good choice for small early cancer that is unlikely to have spread to the axilla. When the suspicion for cancer already in lymph nodes is high, then full axillary node dissection should be performed.

The above are only general guidelines. No two breast cancer patients are exactly alike, and therefore the decision making process is different and unique for each individual person. Regardless of any one else’s advice, you should make the choice that you personally will be able to live with for the rest of your life.

Breast Cancer Time Scale

 Breast cancer is affecting approximately 200,000 women each year all over the world. From those, about 5% will die from the disease. The key factor is early detection. The prognosis time frame to treatment time frame varies on the type of testing done.

Initial testing should be done at home. It is a simple test of palpating the breast in a circular motion from outer to inner patterns. This test is performed weekly so that you are able to distinguish differences. If any lumps or hardness is found an immediate report to the physician should be made to schedule more definitive testing.

Mammograms are performed annually. This is the most effective detection test. Women have been skeptical of this testing method due to it’s discomfort. However, the benefits of knowing outweigh this greatly.

An MRI (Magnetic Resonance Imaging) usually detects breast cancer in the opposite breast of women previously diagnosed with breast cancer.

After either of these tests are performed, a biopsy is performed to determine the nature of it’s source. In other words, to see if the material is cancerous.

One of the more evasive tests is a BDL (Breast Duct Lavage) This test extracts tissue starting from the milk ducts and continues to the nipple through a special pump that covers the nipple. The suction draws out a tiny amount of fluid that tells the physician which ducts to lavage. A catheter is then inserted in the opening of the nipple and saline is used to wash and withdraw more cells. Cancerous cells typically start in the cells that line the ducts. The sample is then studied looking at the cells to determine if they have malignant qualities.

Surgery, chemotherapy, and radiation therapy are common treatments. Results of tests are seen by the doctor within 3 to 7 days. At that point your physician will sit down with you to discuss your unique treatment situation. From there you will learn which method is going to give the best results.

When choosing surgery, consider the act itself one that will go quickly. After, there is still more testing to be done and could even call for additional surgery at a later date.

Both chemotherapy, and radiation therapy require a longer process.

Depending on how much cancer is detected determines the quickness of treatment as well as level of treatment. Cancer of any kind is a curious creature. With the various forms of treatment, more and more women are surviving breast cancer everyday.

Breast Tumor Markers – Are They Useful?

Breast cancer is the most common form of cancer in women and the second leading cause of cancer deaths in American women. In 2009, approximately 194,280 patients are estimated to be diagnosed with invasive breast cancer, and 62,280 with carcinoma in situ. An estimated 40,610 will die of this disease. For a woman of average risk, the lifetime incidence of breast cancer is one in eight.

Serum tumor markers for breast cancer used in the clinic include CA 15-3, CEA (carcinoembyonic antigen), and CA 27-29. All have low sensitivity and specificity, and thus not helpful in detecting early breast cancer. CA 15-3 levels are increased in approximately 5-30% of patients with stage 1 disease, 15-50% with stage 2, 60-70% with stage 3, and 65-90% with stage 4. CA 15-3 measurements are also elevated in 15-20% of women with benign breast conditions, 50-60% with liver disease, 20-70% pulmonary malignancies, 15-60% of gastrointestinal/colonic malignancies, and 40-60% of ovarian cancer cases. CEA is more prevalent in colorectal cancer, whereas CA 27-29 is more specific for breast cancer. These three tumor markers have, however, been validated for monitoring treatment in patients with advanced disease, particularly if the cancer cannot be evaluated with conventional imaging. The American Society of Clinical Oncology recommends the use of CEA, CA 15-3 and CA 27-29 only in metastatic settings, whereas the European Group on Tumor Markers recommends their use in disease surveillance in general.

With the current technology, circulating tumor cells have been found in very few cases of early stage breast cancer. Circulating tumor cells detected in both localized and metastatic breast cancer patients have been associated with worse outcome. Circulating tumor cells may also predict response to therapy.

There is much ongoing research to investigate new biomarkers for early detection of breast cancer. Blood-based markers include cells, DNA, RNA, peptides, sugars, and autoantibodies. Breast-based markers such as nipple/ductal fluid and breast fine needle aspiration (FNA) also include cells, DNA, RNA, proteins, sugars, and autoantibodies.

In the future, it is likely that a combination approach to measure simultaneously multiple markers would be most successful in detecting early breast cancer. Ideally, such a biomarker panel should be able to detect breast cancer in asymptomatic patients, and improve the accuracy of screening mammograms. A reliable biomarker signature may also signify new breast cancer, even in the setting of normal mammogram and physical examination, and would indicate further more intensive diagnostic workup and/or preventive treatment.

There are many causes of benign breast cancer. These are the changes in the breast tissue, any breast infection or injury, and any medicine that are taken that have caused the lumps or the breast pain. Changes in the tissues of a woman’s breasts occur with the fluctuating levels of estrogen and progesterone during the menstrual cycle.

Types of Benign Breast Conditions

There are many types of lumps and conditions in the breast that are benign and it is very important to know them so that you will not have to be unnecessarily alarmed when you find a lump in one or both of your breasts.

    * Fibrocystic changes. This increases the size and tenderness in the breast before the menstrual period.     * Simple cysts. These are just fluid filled cysts that change in size with the woman’s menstrual cycles.

This type of benign breast condition is due to the excessive formation of the milk producing glands and the surrounding breast tissue. This condition will most likely occur in women between the age of 20 and 30.
    * Intraductal papillomas. This condition is characterized by small, wart-like growths in the lining of the mammary duct near the nipple area and women between the age of 45 and 50 are the most likely to be affected.
    * Traumatic fatnecrosis. This condition will occur if a trauma like an injury occurs in the breast and the characteristics of the lumps are round, firm, hard, single, and usually painless.

What You Should Do If You Find A Lump

Once you discover a lump in your breast you should immediately see a physician because it might be a benign breast cancer condition. It is recommended to see a doctor right away if you find the following in your breasts:

    * An area that is distinctly different from another area in either breasts
    * A lump or a thickening near the breast or the underarm that is persistent
    * A change in the size, shape, and contour of the breast
    * A mass or lump in your breast
    * Changes in the feel or appearance of the skin on the breast or the nipple.
    * They are dimpled, puckered, and appear scaly or even inflamed.
    * A bloody or clear fluid discharge from the nipples
    * Redness in the skin on the nipple area or anywhere on the breast

A monthly examination of the breast is recommended to detect any abnormalities that are found in the breast and to prevent any development of benign breast lumps.

With this said, breast cancer is the fifth most common cause of death by cancer. And here’s something I bet you did not know: breast cancer accounts for about 1 percent of cancer deaths in men. Yes that’s right, men can get breast cancer too.

What is breast cancer? Breast cancer is cancer of the breast, which is a malignant growth in the breast tissue of women and even men. There are many risk factors pertaining to breast cancer although there is no set known cause. Some of these risk factors include: a family history of breast cancer, obesity, diabetes, hypertension, and late menopause. If any of these risk factors apply to you, it is important to take extra precaution. Women can do this by doing a monthly self breast exam, or having one performed by a doctor. Another preventative measure is having an MRI performed. MRI stands for Magnetic Resonance Imaging. Another screening method is x-ray mammography, which is most often referred to as a mammogram. The newest form of breast cancer screening is ultrasound computed tomography. This method creates a three dimensional image to help detect cancerous cells. Any preventative measure taken again breast can potentially be crutial to the health of a man or woman.
Along with self examinations, it is also important to look out for symptoms of breast cancer regardless of any risk factors. A common symptom is an abnormal lump in the breast or armpit, along with change in size or shape of the breast. Although those are the most common symptoms, there are several others to be aware of. These are fluid leaking from the nipple, change in shape or size of the nipple, or change in color or texture of the areola. Unusual pain in the breast or armpit can also be a symptom of breast cancer.

If someone is diagnosed with breast cancer, there are several different types of treatment options available today. The doctor will discuss and decide with the patient which type is best for their particular case. Commonly, surgery will be performed where the cancerous tumor is removed, followed by hormonal therapy. chemotherapy, or radiotherapy. Depending on the risk and severity of the tumor, radiation therapy, chemotherapy, hormone therapy, or immune therapy can also be options.
